As a patient at our clinic, you may be requested to complete one or more of the following forms. In an effort to save time when you come in, you can print the form(s) directly from this site. Just click on the link below to open the form you need, then use the File > Print function on your computer to print the form. (You will need to have Adobe Reader on your computer in order to open and print the forms. We recommend downloading only the most recent version of Adobe Reader; the Adobe Reader download is free.)

We request that you arrive 15 minutes early to fill out required paperwork.

Cancellation Policy

There is no charge for appointments canceled at least 24 hours in advance. In order to be fair to others who need appointments, we charge the full fee for cancellations made less than 24 hours in advance, or for no shows.
